HAZARD ID / NEAR MISS / OBSERVATION | HID1
Our Hazard ID / Near Miss / Observation | HID1 form template is easily marked by the form author to confirm the type of event that occurred in order to determine how the information should be processed. Prior to the four full width lines to describe your recordable event are fields for Report Date, Location and Reported By. Following that is an easy-to-use checkbox listing of 18 common categories to help ID root causes of the event. You can go on to document the recommended actions, personnel responsible for completing those actions and a supervisor’s sign-off.
Hazard Identification is part of the risk assessment process. Some of the categories used to ID hazards are physical, biological, ergonomic, chemical, etc. Regardless of whether hazards are identified before, during or after a job, task or event, it is important that they be recorded for the prevention of accidents and injuries from occurring. Subsequently, it is through the reporting and analyzing of potential or existing hazards that effective preventative measures are created for health and safety programs.
